meridianEMR takes health maintenance reminders to the next level in our newest
release of our EMR system. The new Health Maintenance Explorer™ module (HME) is a
real-time process that constantly explores for any patients that meet the criteria of a practice’s health maintenance
rules.
The practice simply creates as many health maintenance “explorers” as they desire, using a set
of rules that are based upon a multitude of data elements in the system.
For example, an (HME)
can be created to track all male patients over the age of 50 that have a high PSA value or a specific diagnosis or are on
a specific medication.
As part of that (HME), the practice also defines which procedures are
to be scheduled once the reminder rules have been satisfied. Not only can the practice define the specific procedures that
need to be scheduled, the chart can be sent to the person in the practice that handles the actual scheduling of the procedure.
Based on a notify setting the chart will appear within a specific number of days before the procedure is due.
For
example, if a patient is to be scheduled for a procedure once they reach the age of 50, the system identifies the patient
and sends the chart to the person in the practice that schedules the procedure - 30 days before the due date. Now the
practice has sufficient time to contact the patient and schedule the procedure prior to the actual due date.
Once
the reminder has been created, the (HME) goes to work. It watches as the practice uses the system and if
any changes are made to patient data that would affect an (HME) rule, the system immediately AND interactively
determines if the patient now meets the criteria - and if so, the reminder and procedure is automatically added to that patient.
The (HME) is always active. There is no need to remember to run any batch processes to find the
patient’s that meet the rules. Of course, at any time you have the capability to use the (HME) to find
any patients in your database that meet any criteria in the reminders you create.
The clinician can also add an
(HME) to the patient directly. As part of the patient’s encounter or while just reviewing the chart,
the physician can decide to insert a (HME) reminder or just order the procedure directly.
You
can view what the (HME) is doing at any time, by simply clicking the (HME) icon on the desktop.
It will display all the (HME)’s that are currently active in your practice.
